Family Advisory Council Application Form

We need your help. We are looking for some energetic and hard working parents, grandparents, aunts or uncles of children who have either used our hospital’s services or who might use them in the future.

We want you. Volunteer to be a part of the Family Advisory Council--an exciting group that helps us to form policies, improve services and make an impact on the work that we do at our hospital. The Family Advisory council allows the hospital to understand what families’ needs are, and our families learn why the hospital works the way that it does. The council responds to hospital department requests for assistance, provides input regarding facility planning and development and serves as an oversight to other family committees and councils. When we include families in the work we do with children, we all win.

Our council meets on our main campus the fourth Tuesday of each month at 5:30 p.m. and active participation is required. If you would like to be considered for a position on the council, please fill out the form below by May 31, 2026.
